All SM helmets are horrid, and no self-respecting SM would ever be seen in one. Therefore just click the 'hide item' option on your Paper-Doll screen.
Nothing can be done about the dresses. Elf clothing was originally designed by Harvey Fierstein, so.
As for appearance of other items such as swords, shields and shoulder pads, any item found anywhere in the RoR world can be slotted for appearance. I do not know of any official guide, but changing the appearance of an item is easy enough to play around with. Some percentage of the player-base at large takes Fashion-Hammer quite seriously. I'm sure one of the Fashionistas will be glad to help out.
