My shamman has Bloodlord weapon with 2 talisman slots. This is probably edited in photoshop or something. Don´t panic and stay cool.

Visual bug most likely since killboard is kinda buggy with the search function not working either atm.

I checked now since killboard is working and yes he has 3 talismans on hes wep idk how, This is sus ngl and you should not be able to have 2 of the same 24 talisman on 1 wep let alone 3 could still be a visual bug but idk.

Is this technically possible in the game? If so, it would be a good option for upgrading weapons without reworking them or introducing new ones. Perhaps as a transitional option between old and new weapons.

Since you're not able to put the same tali in twice this has to be a visual bug

Not necessarily a visual bug, exploits and hacking does happen, people can do weird stuff. File a ticket.

Loading the profile into RoR planner displays two different talis on the weapon, guess it's a visual bug, probably the weapon been previously (pre-patch) equipped with two of the same tali, then after the change in some way it visually "retained" the previous tali, at least in armory...
