Deathblow 2 addon issue, icon in middle of the screen.

Hi all,
Slight issue with the Deathblow 2 addon. The book icon the addon uses is stuck in the middle of my scrreen and I can find no way to move it. I have tried uninstalling the addon etc but no luck.
Any advice?
