What is that? And how to fix that?

You try to equip decimator set?
I thought this bug was fixed. As workaround, check vendors, there are 2 sets for war crests. Buy the other one to earn unlock. Alternatively there is a pve set from public quests, which gives you the unlock as well.

I got that item while playing in a scenario.

The reason is writting in gray in your chat window.

There is a "ward system". You can only equip higher sets when you have obtained the lower level sets first.

You need to unlock the "Recruit ward" first. Check the Armory in the tome of knowledge to see which set items you need to unlock that ward (click on that brown book up there in the middle).

Yeah, and obtaining the lower sets will consume too much time to get them. And one set requires to waste of precious war crests.
