Tactics disappearing

Please can anyone tell my why for the past 2 weeks randomly my tactics will just disappear. If I die one or more are gone, if I log in one or all sometimes are just simply gone and now during gameplay the yare just going away randomly. If I port some are gone wtf?!!!!!! I have now created multiple bars for them so I can switch if it happens but again they are just going away for no reason. Death, logging in, porting or combat one or more are just disappearing. Anyone please help with this!!!
