PQ Deathstone Quarry in Troll Country

Hello,
The PQ Deathstone Quarry (in Troll Country, Order Chapter 5 - around 22100x41000 on map) is frozen in Stage I, it asks to destroy Chaos Rift 2/3, but there is no more rifts around. A few days ago I kill the 3 Rifts, but only 2 counted and it not respawn, than the PQ is not reseting.
Thanks for your attention, i'm doing this PQ's for the "Nothing in Vain" Epic Quest.
