Had a scenario fight today and got several times being instakilled. Match level is T1. And checking the chat log it looked like I killed myself several times.
In the screenshot you can see that this happened to me four times. And the strange thing is that I played as an SM and not a sorcerer.

Maybe your effects are off and you cant see the explosion in this sc? Just a guess

Looks like Logrins Forge scenario. This one has a mechanic, that probably killed you

They added a Khaine's Embrace mechanic to Logrin's Forge for some reason. Turn on volume and listen for the war horns and run away from the flags asap when you do.
