Questing with a lower leveled player.

When I group up with another player who is 6 levels under me, neither of us get our quest objectives achieved. Is it the level gap that is causing this or is there a problem that needs reporting in greater detail?

Just the level gap. You need to be within 4 of each other.

Not Questing, but I think you should be both capable to go in the lvl 16+ Dungeon in Altdorf, the Sewers.
High Level ones are tuned down to lvl 19 and can make a party with low level chars.
XP should come to the lower level player.
Not the best way but it should work.
