Range tracking, how does it work?

I'm completely lost on this, does anyone have any idea how the game currently is supposed to track and calculate range for abilities?

The reason I'm asking is because I might be missing something huge related to this screenshot, but I have no idea what it could be. And as such I would like input from anyone that knows for sure what's going on. If you are only guessing please refrain from posting, I struggle enough making sense of this with my own wild guesses, every time it makes sense someting else pops up that makes no sense.

The only actual info I have to go on is this old patch, but even based on this the ranges don't make sense at all to me.

I try to eplain the image...or at least what i understand

There are two type of "range", Single Target or AOE

Frome the left, you have you 0ft REAL, then the assumed 5ft(the assumed as a REAL 5ft, keep reading), from here to the right you have the teorycal distance from "0 ft" (assumed) to 100ft Single target.

What the image is telling is about "5ft" ST, which is at 15ft from the real 0ft instead on the assumed 5ft.
That is what is "wrong".
AOE starts at 20ft, take that as a reference and go to 0ft, you can split the space between in 4 (5ft each).
The assumed 5ft doesn't match with the real 5ft.
That's why the mobs always hits you "from far", been most of them melees¿?
