Skull belt Festenplatz Pillagers Midstrap or any belt from the T1 green armor Renown vendor and it drops from any mob in T1
"eye" belt is a level 5 blue item or level 3 purple item that drops from random T1 mobs or rewarded in T1 PQs

Vainglorious quest gives you all those items you see in the guide that are not part of the actual sets.
