When you cast a hot on someone, you don't heal them immediately. It only places an enchantment or blessing on your target, which causes the heal.
It is similar to offensive "on hit" effects, which only proc off direct damage.

It depends on how you describe buff.
Improving some skills or fixing serious bugs results both in a better performance.

Shamans are 3d pick healers nothing has changed
Even x2 dok is better says it all really

Gear wise, full X, or at least until the 5% heal crit bonus (for a total of +7%, 2% from shoulders). So collect gear and stay in it until you can completely UG to the next set with your 7% heal crit.
Here is the general build that I think works in all situations:
RoR.builders - Shaman. Note that You Got Nuthin pretty much only works on dps AMs and BW, as dedicated healers will have a lot more disrupt vs your chance to apply. The fourth tactic I run is Run Away! for solo and Discipline for party/WB/keeps, though many here don't favor more than ... I think 750? WP. I like more. You'll find your spot.

Sadly, dps shamans can't spec "You Got Nuthin" without using a trash mastery spec. Am/sham without snare puddle are too easy to kill.
