Without NPCs the defenders will just leave via the back when the door falls like they did on live

They will defend keep if hawe enough ppl,if they won't defend their choose.

Wouldnt it result in a better fight tho?
Also due that the flags are spread over the keep area so people have to split up and fight at different locations = more organization and quality of play.
Atleast thats what i think could happen.

i am not a fan of having all these buggy respawing champion mobs inside keeps i see no purpose for why they should exist bar giving defenders an advantage they dont need

It might be worth a try to see if mentality differs from live. I just have so many memories of everyone jumping out of the back as attackers go in the front. Some of testpigs videos are still up on youtube of him taking the piss out of war as they run into a defended keep to find it empty and just a flag to click.

More recently, since postern fall, courtyard isn't defended.
I wonder why Ppl would defend an empty keep without their lord support.
