Spawning in the keep while outer door is tagged

Currently, it is really easy for defenders to get inside the keep when only the outer door is tagged. With state of realm showing door status and the plethora of PVE routes that can get you near the keep, it is impossible for attackers to prevent defenders from getting inside while only the outer is hit.

In a situation where realm numbers are even on both sides, it is impossible to make a push due to how easily defenders can get inside a keep as soon as they see the outer being hit.

When the inner is hit, the attackers actually make effort to deny entry into the keep. It should be the same for when the outer is hit. Conversly, defenders who weren't already in the keep before the siege started should have to make effort to get inside the keep.

Actually I would be totally okay with this IF (and only IF) there was some kind of zone-wide counter mechanic to zerging down the keep that forced the attackers to pay attention to the rest of the zone instead of solely concentrating their forces at the keep.

It was attempted before by strengthening the lord based on how many flags you held but that didn't work out.

actually when outer is under 50%, defenders can't use it anymore.

Defenders can still just suicide on the ram for any sort of insignificant damage while outer is still up, and when you have 200 people in the keep, it doesn't matter how fast they die, they did their part and they can just continue doing it until the ram is dead.

Have reviewed the last patch notes (last 2 months) and didnt find this...could tell when this changed?
