Remove the ability to interact with outside doorways while in combat

The whole concept of a doorway being in some superposition state of being both opened and closed depending on whether you are friend or foe is corny and leads to cheese, remove it.

Keep fights are tedious enough already. If you add this change you will kill off the whole fighting around posterns.
