Return of Reckoning

There are two epic quests available in T2 tier:

1. Boss Huntin' (Greenskins Ch.5) - rewards Epic Shoulders (r17)
Problem : The quest stops sbruptly in Greenskins Ch8. It disappears from Quest List and there's no aother NPC giving it.
Reward :Type this in /s chat: <LINK data="QUEST:49074" text ...

This is maddening.

Quest: Sanctify
Location: Empire Ch.12, Deathmire Forrest
Problem: Unable to find The Flame needed to complete the quest - please see the images below. EDIT: The Flame is on some altars, but it's not clickable.
